~ Heart Failure Educational ~

As part of our American Heart Association–certified Heart Failure Program, Tufts Medicine Care at Home provides evidence-based Heart Failure education modules for all staff who care for patients living with heart failure.

These education modules are designed to strengthen early recognition of heart failure changes, improve patient sef-management, and support our ongoing efforts to reduce preventable hospitalizations. Each module focuses on a key clinical or patient-care topic and includes resources tailored to multiple disciplines.

Clinicanas are required to complete at least one Heart Failure education module as part of their annual education requirements. Additional modules are available and encouraged to further enchance clinical knowledge and best practice.

Below, you will find links to the available Heart Failure education modules, along with any related resources and required assessments. Completion of the reuired module will be documented in your education record.​
