~ Monthly Heart Failure Educational Series ~

As part of our American Heart Association–certified Heart Failure Program, Tufts Medicine Care at Home provides monthly, evidence-based training for all staff who care for patients living with heart failure.

This educational series is designed to strengthen early recognition of heart failure changes, improve patient self-management, and reduce preventable hospitalizations. Each month’s training focuses on a key clinical or patient-care topic and includes resources tailored to multiple disciplines.

Below, you’ll find links to the current and previous training modules, along with any related materials and required assessments. Completion of assigned modules is mandatory and will be recorded in your training file.
