VNA OASIS Influenza Data Collection Period
1) Data collection for the flu vaccine begins Oct 1st – March 31st
2) At every SOC/ROC you must ask:
a) When did you receive your last flu vaccine?
b) Have you ever received the pneumonia vaccine?
3) Document where when and where received on the Immunization form
4) Any DC or Transfer OASIS done Oct 1 - March 31st should be marked yes for M1041 Influenza
